Norfolk Safeguarding Child Practice Review published today
A Child Safeguarding Practice Review has been published today into the death of a teenager, identified as AO - Jasmine.

Chris Robson, Independent Scrutineer of the Norfolk Safeguarding Children Partnership, said:
“Our thoughts and sympathies remain with Jasmine’s friends and family. It is clear from the comments of those that contributed to this review that she was a vibrant and much-loved young woman, and her loss is felt deeply by those who knew and supported her.
Despite the concerted efforts of many people to keep Jasmine safe, this review highlights how difficult it can be to protect teenagers who have experienced past trauma, as they move towards adult life and try to live independently.
The review recognises that there was a wide range of support offered to Jasmine and that professionals were able to build strong relationships with her that did have a positive impact. Sadly, this was not enough to stop the misadventure that led to her death.
As a partnership we value the work of the lead reviewer in this case and accept her recommendations. We have already done significant work to strengthen our work with fathers and enhance the support for young people at risk of exploitation. An action plan is in place and we will be closely monitoring its implementation.
